Tesamorelin mechanism of action
A small chemical modification gives a familiar hormone signal longer to work. The response includes growth hormone pulses and a rise in IGF-1.
GHRH and the pituitary
Tesamorelin copies the full 44-amino-acid human GHRH signal and carries an addition at one end of the chain. That addition resists DPP-IV, an enzyme that otherwise deactivates the signal. The non-clinical pharmacology paper on TH9507 tested slower breakdown in rat, dog and human plasma in vitro, alongside animal experiments. Those plasma tests took place outside the body.
At the pituitary, tesamorelin binds the GHRH receptor on cells that make growth hormone. The receptor sets off internal signalling involving cAMP and PKA, leading to hormone production and release. Human exposure-response modelling followed tesamorelin, growth hormone and IGF-1 together.
The engineering impresses me for its economy: protect a message already recognized by the gland, then observe how the gland answers. Even with the added protection, the drug has a short stay in blood.
Overnight hormone sampling
The study in 13 healthy men followed the response over 2 weeks. Mean overnight growth hormone increased by 0.5 ug/L, and IGF-1 rose by 181 ug/L. Growth hormone secretion remained pulsatile: the gland released it in bursts.
This detail matters to the tesamorelin mechanism. Tesamorelin stimulates hormone made inside the body; a growth hormone injection supplies the hormone itself. The human pharmacokinetic and pharmacodynamic analysis also modelled episodic secretion. I’d put that rhythm ahead of the vague expression “hormone boost,” which leaves out the behaviour the researchers measured.
The healthy-men experiment addressed physiology over a short period. It offers no long-term fat-loss result for the general population.

IGF-1 and visceral fat
Growth hormone stimulates the liver to produce IGF-1, a growth factor. Together, the GH/IGF-1 system helps mobilize stored fat. In the pivotal HIV trial, IGF-1 increased 81.0% while visceral fat decreased.
To me, the IGF-1 result explains both the interest and the caution. A growth factor is part of the biological response being sought, and its elevation also raises safety questions. The research does not establish the trial’s increase as a desirable personal target. Side effects and longer-term uncertainties need to travel with any account of the mechanism.
Adiponectin in the small human study
Adiponectin is a hormone released by fat tissue. In the 2-week study of 13 men, growth hormone pulses and IGF-1 increased, yet neither total nor high-molecular-weight adiponectin changed significantly.
I trust this awkward outcome more than a perfectly tidy explanation. An explanation that makes every downstream marker improve is too tidy for this experiment. The gland clearly responded, while this particular metabolic marker stayed statistically unresolved.
Inflammatory markers in the HIV studies
A different analysis in people with HIV and excess abdominal fat found lower tissue plasminogen activator antigen and a modest increase in adiponectin. Changes in inflammatory markers correlated with the amount of visceral fat lost.
I read those findings as a reason to study the changing fat tissue closely. The association supports a connection with visceral-fat reduction; it cannot assign every change to a direct anti-inflammatory action of tesamorelin. The research page follows that question into liver tissue and CT measurements.
